Sleep Apnea, With Dr. Stephen Poss, Dentist, Franklin, Nashville, Brentwood, Tennessee

As more people and physicians are starting to become more aware of sleep apnea, there are some signs that show up early on that might indicate someone has sleep apnea, says Dr. Stephen Poss, a dentist in Brentwood, Tennessee.

Loud, persistent snoring, being exhausted and a sleep partner who hears gasping and choking at night are all early warning signs of sleep apnea.

Dr. Poss says that just because you snore, doesn't mean you have sleep apnea but that 35% of snorers have some form of sleep apnea.  It's also a strong indication that you should meet with your physician to get it evaluated further by way of a sleep study.

Process of Treating Sleep Apnea, With Dr. Stephen Poss, Practical Sleep Solutions, Brentwood, Franklin, Nashville, Tennessee

For patients with mild to moderate sleep apnea, Dr. Stephen Poss, a dentist in Brentwood, Tennessee with Practical Sleep Solutions, will get a copy of the patient's sleep study and prescription from the referring physician.  He does a complete TMJ exam to make sure the jaw is suitable for an oral appliance and a 3D scan to make sure there's no airway obstructions.  He will also start with an impression of the patient's mouth to get an accurate bite.

Dr. Poss communicates with the referring physician so they know what is going on with their patient.  After the patient gets their oral appliance, they will then fill out questionnaires on how it's working at home.  The bed partner will also complete a questionnaire about the patient's sleep habits using the oral appliance.  Dr. Poss likes to see how things are going as it relates to the patient's snoring and if their energy level has increased, among other things.  He likes to track this for the first few weeks and make changes as needed.  Lastly, he will request a final sleep study from the physician.

Physician Referral Process, With Dr. Stephen Poss, Practical Sleep Solutions, Brentwood, Franklin, Nashville, Tennessee

For any physician referring a sleep breathing disorder patient to Dr. Stephen Poss, a dentist in Brentwood, Tennessee with Practical Sleep Solutions and Poss Dental Designs, he always conveys to the patient that a CPAP is the gold standard for treating sleep apnea.  Dr. Poss also knows that some people aren't great candidates for the CPAP, so he follows the protocol set forth by the ACSDD, the Academy of Clinical Sleep Disorders Disciplines.

After the requested sleep study by the physician is completed, along with a written prescription brought in by the patient, Dr. Poss will do a complete TMJ exam, an oral exam and a periodontal exam to make sure the teeth and structures are healthy enough for an oral appliance.  Dr. Poss only uses FDA-approved oral appliances.

After several calibrations, as soon as the symptoms dissipate and the breathing and snoring is better, Dr. Poss will send a letter to the referring physician to request a follow up sleep study.  Staying in the loop with the referring physician is most important, according to Dr. Poss.

After the oral appliance is placed and adjusted and verified by the physician that it's been effective, Dr. Poss will see that patient twice during that first year and then once each year thereafter.  Dr. Poss also works with all major insurance companies, including Medicare, to make sure a good amount is covered after the patient's deductible has been met.

Oral Appliances for the Snoring Patient, With Dr. Stephen Poss, Practical Sleep Solutions, Brentwood, Franklin, Nashville, TN

Snoring can sometimes get overlooked and some people have what's called primary snoring, without sleep apnea.  This can affect the bed partner's sleep as well and Dr. Poss, a dentist in Brentwood, Tennessee with Practical Sleep Solutions, says that the main reason his patients come in about the snoring is so that they can start sleeping in the same room again as their bed partners.

There are over the counter oral appliances that patients can wear but Dr. Poss says "they're not even in the same ballpark."  The problem with the over the counter appliances is the fit, as they don't stay in place and because they're fixed in one piece, they can cause TMJ problems, because the jaw isn't able to move during the night.  The oral appliances that dentists make allow for jaw mobility.  Dr. Poss says the over the counter products often do a lot more harm than good.
